non-retirement mutual fund account and/or money market account and received
taxable dividend income, interest income or capital gain distributions, or
sold shares of a mutual fund. (Read
our important tax information brochure). If your account earned less than
$10 in interest you will not receive a 1099.

account and took a distribution or re-characterized IRA assets.

If you made a
contribution (including rollovers, conversions, recharacterizations,
